- Ensure optimum therapeutic outcomes for targeted beneficiaries through improved medication use
- Reduce the risk of adverse drug events
- Facilitate cooperation between pharmacists and physicians
- Demonstrate the resources to manage the population
- Provide services by pharmacists or qualified providers
- Distinguish between services in ambulatory and institutional settings
- Coordinate care with planned care management for chronic care improvement
